Product name,
Quantity,
Price,
Shipping address,
Desired method of shipping (UPS ground, Next Day Air Saver, UPS 2nd Day, or UPS 3-day),
Credit Card number and Expiration date,
Email address, and
a phone number where we can contact you if we have questions.
Tax will be added for all orders shipped to an address within the state of Arizona.
